== Prognosis == The prognosis is generally poor with survival typically measured in months. The median survival time of patients without treatment is four to six weeks. The best prognoses are seen from NM due to breast cancer with the median overall survival of no more than six months after diagnosis of NM. Death is generally due to progressive neurological dysfunction. Treatment is meant to stabilize neurological function and prolong survival. Neurological dysfunction usually cannot be fixed but progressive dysfunction can be halted and survival may be increased to four to six months. It occurs in approximately 3-5% of cancer patients. The disease is usually terminal and if left untreated, the median survival is 4–6 weeks whereas if treated, the median survival can increase to 2–3 months. Treatment will be more effective if it is done on the primary tumor before it metastasizes to the brain or spinal cord. Patients with leukaemia achieve better results compared to patients with solid tumours who have undergone treatment. It was found that 75% of patients stabilize or improve over several months as opposed to 25% of patients who do not respond and have progressive disease. But despite initial improvement, most patients survive only a few months. ==== Malabsorption ==== Individuals with pernicious anemia do not have the ability to produce intrinsic factor. Individuals who lack intrinsic factor have no ability to absorb vitamin B12. The lack of intrinsic factor is most commonly due to autoimmune gastritis, which causes an autoimmune attack on the parietal cells that create it in the stomach. Antacid drugs that neutralize stomach acid, as well as acid-suppressing agents such as proton-pump inhibitors, can inhibit the absorption of vitamin B12 by preventing its release from food in the stomach. Other causes of B12 malabsorption include bariatric surgery, pancreatic insufficiency, obstructive jaundice, tropical sprue, celiac disease, inherited intrinsic factor deficiency, and radiation enteritis affecting the distal ileum. Age is also a contributing factor: elderly individuals are often achlorhydric due to reduced parietal cell function in the stomach, increasing their risk of vitamin B12 deficiency. ==== Elimination ==== Modafinil is eliminated 90% via metabolism and 10% via renal excretion. Modafinil, given as a single dose in radiolabeled form, is excreted 80% in urine and 1% in feces by 11 days post-administration. Urinary recovery as the major metabolite modafinil acid has been found to be 35 to 51% of a dose. Less than 10% of modafinil is excreted unchanged in urine. The elimination half-life of modafinil is in the range of 12 to 15 hours There is individual variation in this parameter depending on sex, cytochrome P450 genotypes, liver function, and renal function. The individual enantiomers of modafinil, armodafinil and esmodafinil, have substantially different pharmacokinetics due to differing elimination profiles. Both armodafinil and esmodafinil are eliminated in a monophasic manner. However, armodafinil has a half-life of 10 to 17 hours, while esmodafinil has a half-life of 3 to 5 hours (3–4 times shorter).
